Conference Call Sign-up Form

Here is an opportunity for you to speak to Ben on the phone!

Ben has graciously agreed to participate in a telephone conference call with friends and fans on July 13, 2008.

The conference call with take place at 1:00pm Pacific time (Ben time!)  That's 4:00pm Eastern time, and in the evening in European countries.

If you sign up to participate, you will be given a dial-in phone number, a passcode and an attendee code.  At the designated time, you will call the

phone number and follow the prompts to enter the passcode and attendee code.  Other friends and fans will also be dialing the same phone number,

so everyone will be on the same conference call together. Ben will also dial in to join the call.  The call will be moderated by Jane (the webmistress

of this site), and Jane will give each person a chance to speak, to ask Ben questions or just to chat with Ben.  Everyone else on the call will be able

to hear as each person takes their turn talking to Ben. Each person will have approximately 3-5 minutes to talk to Ben.  The conference call will last

approximately one hour.

The conference call will be recorded.  However, any audio clips made from this call will contain only Ben's voice when he is speaking.

Jane will edit all audio clips to remove any identifying voices or names before placing the clips on this web site at a later date. Also, other

participants in the call will hear only your first name and your country. Any other information (such as your last name or your state) will not

be revealed to anyone other than Jane and Ben. Your phone number will not be seen by other attendees, even if they have caller id.

When you sign up for the conference call, you will be able to choose whether you want a turn to talk to Ben, or if you just want to listen live

to the entire conference call. There will be a limit on the number of people who will be given a turn to talk to Ben, due to time constraints,

so if you want to talk to him, be sure to sign up early. There is no limit on the number of people who can listen to the call.

There is no cost to join this conference call.  However, the dial-in phone number will be to an area code in the middle of the United States.

If your phone service requires that you pay for phone calls to this area, you will of course see this phone call appear on your next telephone

bill.  The conference call service does not add any fees or surcharges of any kind. The call will appear on your phone bill just as if you had

called on a Sunday to a friend in the middle of the United States and talked for a little over an hour. Participants will need to dial-in 5-10

minutes before the conference call begins to be admitted to the call.

Ben has said that no topics are off limits. However, Jane would like to remind everyone that courtesy and discretion will be appreciated!

If you would like to participate, please fill out the form below.  All fields are mandatory. You are also welcome to tell other Ben Murphy fans

about this call and encourage them to sign up.  Sign-ups will be accepted until midnight on July 10.

On July 11th, everyone who completed and submitted the form below will receive an e-mail message containing the dial-in phone number,

a passcode, and an individual attendee code.  Your attendee code will be unique to you and should be kept confidential.  Only people who
